We appreciate your selection of Diamond Women's Center for your OB/GYN care. We look forward to seeing you.
To expedite your registration, we ask that you complete our downloadable Health History and Billing Information forms (available on our Patient Forms page) and bring them with you at your appointment time.
Please bring your insurance card and completed forms. If these forms are not completed before you arrive, your appointment will be delayed. Being prepared really helps with the flow of patients in our office. We request this information from all patients to provide you with the best medical care possible. These forms are necessary because our doctors request your most recent health history and we are required to update your insurance information annually.
As a new patient coming to our clinic, it is important to let you know that any records you bring to our clinic cannot be copied or transferred from us to another office or hospital. If you want copies, please make copies for yourself before giving your records to our office.
